Abstract
The 2021 ESC (European Society of Cardiology) and EACTS (European Association for Cardio-Thoracic Surgery) guidelines for the management of valvular heart disease are an update of the 2017 guidelines. The most notable new features include placing the patient at the centre of the assessment and diagnosis, and the importance and relevance of decisions made by the heart team. They stress that certain valve interventions should only be performed in specialist heart valve centres, and that these should have permanent cardiac surgery services and dedicated outpatient teams in heart valve clinics. There are new recommendations on the management of the left atrial appendage in patients with atrial fibrillation, on the indication for the treatment of aortic, mitral, and tricuspid valve disease, on the type of treatment for aortic stenosis and secondary mitral regurgitation, and on the anticoagulation treatment in patients with biological prostheses.
