ABSTRACT Severe mitral stenosis for non-cardiac surgery is a challenge to anesthesiologists. General anesthesia as well as regional anesthesia have their own limitations. We present here a case of abdominal hysterectomy with ovarian cyst in a patient of severe mitral stenosis managed with graded epidural anesthesia who had heart failure in the post-operative period but was successfully managed by continuous positive airway pressure.
