Cholesterol granulomas are rare and benign lesions that can be found anywhere in the body These lesions can be seen indifferent locations in the temporal bone such as in the petrous apex middle ear and mastoid We present a case report of a petrous apex cholesterol granuloma in a years old male with an atypical peripheral facial palsy sensorineural hearing loss and tongue paresthesias CT and MRI studies are essential for the differential diagnosis and the selection of the ideal surgical technique to be performed Due to its safety and little morbidity a trans sphenoidal endoscopic approach was made nbsp
