Abstract
Over the years an increasing number of orthopaedic rehabilitation patients were admitted to nursing home de Riethorst in Geertruidenberg (the Netherlands). For efficiency reasons we developed a standardised rehabilitation follow-up plan for orthopaedic inpatients, which is presented in this article. De laatste jaren is het aantal orthopedische revalidanten dat opgenomen werd in verpleeghuis de Riethorst te Geertruidenberg fors toegenomen. Om het revalidatieproces zo doelmatig te kunnen volgen, werd voor de orthopedische revalidanten een gestandaardiseerd follow-up plan ontwikkeld.
