Abstract
Older adults in skilled nursing facilities are often complex and at high-risk for polypharmacy and adverse medication reactions. To address these issues, an important step is to be able to identify medications that are high-risk for older adults. The goal of the session was to improve nursing facility interprofessional team awareness of polypharmacy and high-risk medication use for older adults. We aimed to address the core component of “medications” in the 4Ms framework of geriatric care for Age-Friendly Health Systems through collaboration with skilled nursing facility team members.
