Abstract

Social work discharge planning services as part of a Geriatric Consultation Team were compared to Traditional Social Work Discharge Planning to patients aged 75 years and older. Findings indicate a more effective pattern of discharge planning through the Team. Earlier identification and evaluation of functioning, leading to a comprehensive view of the elderly, resulted in utilization of more social-health services and reduced early recurrent readmissions.
