Abstract
胃癌胆嚢転移は非常にまれであり，本邦では過去11例が報告されているに過ぎない．症例は66歳の女性で，体重減少，食欲不振を主訴に前医を受診，4型胃癌の診断にて当科を紹介された．精査にて非治癒因子なく，TS-1/CDDPによる術前化学療法施行後，胃全摘・脾合併切除術と予防的胆嚢摘出術を施行した．肉眼的に切除胆嚢に病変を認めなかったが，病理組織学的に胆嚢筋層に広がる胃癌細胞を認めた．直接浸潤や腹膜播種からの浸潤所見なく，胆嚢転移の診断となった．病理組織学的検査所見はpor2，T2（SS），ly1，v0，N1，H0，P1，CY0，M1（胆嚢），Stage IV，術前化学療法の組織学的効果判定はGrade 2であった．食欲不振により補助化学療法を開始しえないが，術後1年を経過し無再発生存中である．同時性胃癌胆嚢転移は非常に珍しく，また術後1年以上の無再発生存もまれであるためここに報告する．
