Abstract

Miliary pulmonary coccidioidomycosis is a feared complication of primary coccidioidomycosis (1o cocci), with high morbidity and mortality.1 This disorder is seen in about 4 percent of patients hospitalized with 1o cocci.2 Miliary coccidioidomycosis usually occurs early in the symptomatic course of 1o cocci, but may also occasionally be a late complication of chronic progressive coccidioidal pneumonia (CPCP) or extrathoracic, coccidioidomycosis (eg, coccidioidal synovitis). This syndrome is believed to reflect massive, blood-borne dissemination to all lung fields (Fig 1), as well as other body sites.
