Abstract

In his commentary1 on the report of Lange et al.,2 Guerrini emphasizes that heterotopic tissue cannot be considered just indolent and may have normal function as well as the potential to generate epileptic activity. He points out that temporal lobectomies sparing deep heterotopic nodules were unsuccessful; raising the suspicion that not just the epileptogenic zone but also the symptomatogenic zone might correspond to the heterotopic tissue. We have some concerns about this statement. The epileptogenic zone is a theoretical concept that describes the area of cortex that is indispensable for the generation of epileptic seizures.
