Abstract

Gunasekaran Sengottuvelu, MD, DM, Ravindran Rajendran, MD, DMChennai, IndiaA 58-year-old-man was referred to us after throm-bolysis for acute inferior wall myocardial infarction.A coronary angiogram showed intraluminal fillingdefects extending from the proximal to distal rightcoronary artery (RCA) suggestive of spontaneouscoronary artery dissection (SCAD) with thrombus(Fig. 1A, Online Video 1). He was managed withintravenous tirofiban infusion along withoral antiplatelets. A check angiogram after 4 daysshowed mild clearing of the filling defects(Fig. 1B, Online Video 2). Optical coherence to-mography run from the distal to proximal RCAshowed dissection flaps (asterisk) along with orga-nized and recanalized thrombus (hash mark)
