Abstract
An 87-year-old female presented altered mental status and left hemiparesis. A cerebral angiogram showed abrupt cutoff of the right internal carotid artery. Mechanical thrombectomy was performed. Postoperative transthoracic echocardiography revealed a free-floating ball thrombus (FFBT) in left atrium. Surgical removal of the thrombus was recommended, but, due to the patient's poor general health status, she was treated with anticoagulation therapy. Her clinical course was uneventful.
