Abstract

A 40-year-old male patient reported to the trauma service of a hospital with pain, difficulty opening his mouth, and facial edema after attempting extraction of tooth 18 approximately 1 week ago. The cone beam tomographic examination showed “bone fractures, with fragment displacement, in the maxillary tuber involving the palatal bone, pterygoid hamulus and lateral lamina of the pteroid process of the sphenoid bone. Tooth 18 is extruded and moved to the palate next to the bone fragment of the tuber, palate, and pterygoid process of the sphenoid. This condition promotes discontinuity of the cortical bone of the floor and posterior wall of the maxillary sinus, of the floor and lateral wall of the nasal fossa, with bucco-sinusal and bucco-nasal communication.” The surgery was performed in a surgical center with general anesthesia and nasotracheal intubation, involving repositioning of the fracture and fixation with wire, plate, and screws. The extraction will be performed later.
