Abstract

Summary A radiological and pathological review is presented of thirty-five cases of non-atheromatous renal artery stenosis seen at St Mary's Hospital in recent years. The majority of the cases were considered to be due to fibromuscular hyperplasia of the renal artery, but cases of perimuscular fibrosis, of congenital hypoplasia, and of extrinsic pressure bands were also encountered. Pathology of the constricting lesion was available in seven cases. The final histological diagnosis in these cases was medial fibromuscular hyperplasia (two cases), perimuscular fibrosis (two cases), localised congenital hypoplasia (one case) and extrinsic pressure bands (two cases). Intimal fibrous thickening was present in three cases, but always in association with other lesions. In the cases due to extrinsic pressure bands histology provided evidence that they were associated with the lumbar sympathetic chain.
