Abstract

The present study was designed to investigate the relationship between human fetal and maternal blood bicarbonate levels during labor and after increasing and decreasing maternal bicarbonate levels. The relationship between fetal bicarbonate and fetal heart rate was also studied. A positive relationship between maternal and fetal base excess was found in the first stage of labor. The maternal base excess increased significantly in the second stage of labor without a concomitant rise in fetal level. A significant negative relationship was found between fetal base excess and fetal heart rate. Maternal infusion of sodium bicarbonate increased both fetal and maternal standard bicarbonate levels. The infusion of ammonium chloride decreased bicarbonate in all 4 mothers and 2 of the 4 fetuses.
