Abstract

To identify family physicians' perception of the primary care collaborative memory clinic (PCCMC) care model. Physicians with access to a PCCMC completed a survey to assess their satisfaction with various aspects of PCCMC care delivery (n = 78). Interviews were conducted with a purposeful sample of physicians (n = 20) to gather their opinions on the strengths, gaps in care and benefits associated with the PCCMC model. There was support for this care model in providing timely quality dementia care at a primary care level, making it easier for referring physicians to manage challenging aspects of dementia care. PCCMCs provide a significant opportunity for supporting physicians to manage the care of persons with dementia within primary care practice.
