Abstract

The ameloblastic fibro-odontoma (AFO) is a subtype of odontoma according to the World Health Organization (2017). It is considered a mixed benign odontogenic tumor with specific clinical, radiographic, and histopathologic features. We report a case of the AFO in an 11-year-old boy with complaints of not erupting the lower first right molar and slight increase of mandible volume. The patient presented discrete facial asymmetry, hard swelling, and painlessness in posterior region of mandible in right side. The imaging examinations showed an extensive radiopaque mass with delimited circumscribed radiolucent halo with cortical bone expansion of 3.0 × 2.5 cm × 2.0 cm, preventing eruption of permanent tooth U46. Histopathologic examination revealed presence of odontogenic epithelium and of ectomesenchyme associated with dental structures. Enucleation of the lesion was performed, preserving the impacted permanent tooth and allowing its eruption process. No recurrence was observed during follow-up of 6 months.
