Abstract

A method of raising a modified extended latissimus dorsi (ELD) flap is described as autologus tissue or in combination with implants for breast reconstruction. The method is designed to reconstruct breasts in the normal anatomical plane, anterior to the pectoralis major muscle. The flap combines muscle, fat, fascia, and skin and it is used in immediate and delayed reconstruction. A safe and easy method of isolating the pedicle and islanding the muscle is shown and a technique of quilting the donor site is shown to eliminate seroma formation.
