Abstract
What is the model for integration in the UK? There is much rhetoric about joining up health and social care to address more than the physical needs of patients. To cite Maslow’s (1943) theory of the Hierarchy of Needs, there are many factors that play into an individual being ‘well’. These include safe and secure housing, education and employment, friendship, and a feeling of belonging. The integration of health and social care provides the opportunity for a holistic view of individuals, to ensure that they are best equipped to maintain their psychological and physical health, wellbeing, and safety. The Greater Manchester Health and Social Care Partnership (GMHSCP) aims to achieve an integrated system as a result of the Devolution Agenda (HM Treasury and Greater Manchester Combined Authority (GMCA), 2014). The role of the community practitioner is pivotal to the devolution vision being achieved, as community practitioners treat, observe, and truly understand the community around them. This article will explore the process and journey Greater Manchester has undertaken, the programmes’ aims and challenges, and the opportunities it affords community nurses.
