Abstract

Objective: To evaluate short-term effects of tolvaptan in patients hospitalized for refractory heart failure(HF). Methods and Results: Four patients(mean age 74.5+11.5years, male 75%, ischemic etiology 75%, left ventricular ejection fraction 36.5+12.0%) who received oral tolvaptan(15mg daily, 7 days) for refractory HF were observed before and at day 7. All patients resisted standard therapy including diuretics, carperitide, and inotropic agents. Tolvaptan reduced body weight(57.1±4.5kg to 54.2±4.9kg), though this difference was not significant(p=0.57).There were no significant difference between before and at day7 serum creatinine level(1.6±0.4 vs.
