Abstract

Heart failure (HF) is a major driver of serial hospitalisations. Rapid-access clinics (RAC) have the potential to reduce inpatient HF admissions. A pilot HF RAC model of follow-up was established at a metropolitan hospital in August 2020, targeted at early detection of re-congestion in the early post-discharge period, alongside education and medical optimisation. We measured the performance of this clinic over its first 5 months.
