Abstract
The Affordable Care Act requires all hospitals to run a Quality Assessment and Performance Improvement (QAPI) program in order to participate in Medicare. But although hospitals are required to maintain a system for reporting patient harm events, far from every event is reported. Hospital incident reporting systems capture less than 15% of patient harm events, according to a 2012 report from the U.S. Department of Health & Human Services. The same is true for medication-specific events, noted Lynn Lee, PharmD, Medication Safety Pharmacist for United Health Services.
