Abstract

Bachmann's bundle is a common site of possible epicardial connection in addition to coronary sinus, Marshall ligament and septopulmonary bundle. Hereby, we presented the image of epicardial ablation for recurrent perimitral macroreentry after endocardial ablation. Endocardial mapping revealed discontinuous activation pattern. Epicardial mapping demonstrated highly fragmented signals at the opposite site of presumed endocardial isthmus. Epicardial mapping and ablation may improve procedural success in selected patients with accurate definition of reentrant circuit despite effective endocardial lesion formation.
