Abstract
For JDMI partner hospitals (UHN, SHS, WCH), all diagnostic imaging reports are electronically transmitted to the organization’s Hospital Information System (HIS), which each affiliated physician has access to. Prior to April 2017, imaging reports were also delivered according to the physician’s preferred delivery method, either by FAX or MAIL. JDMI encountered numerous privacy and patient care incidents as a result of various issues related to faxing reports to physicians. To prevent future incidents from occurring with faxing, JDMI stopped its faxing service to internal physicians.
