Abstract

PurposeThe optimal treatment modalities of ruptured cerebral arteryaneurysm are still controversial. The objective of this study is toanalyze the outcomes of patients with ruptured cerebral arteryaneurysms treated by endovascular intervention in North east ofIran.MethodsFrom October 2012 to January 2018, 447 patients with 447ruptured cerebral artery aneurysms received endovascular inter-vention including simple coiling, stent-assisted coiling, balloon-remodeling and coiling, andflow diverter stent in Mashhadhospitals.Results447 patients included in this study, mean age of patients were54.3±13.7 years. 302 patients (67.6%) had good WFNS grade(WFNS 0–2) at admission and 145 patients had poor WFNS (3–5).The location of 381 (85.2%) aneurysms were anterior circulationand 66 (14.8%) were posterior. 340 (76.1%) were treated by simplecoiling with or without balloon remodeling, 84 (18.8%) by stent-assisted coiling and 15 (3.4%) byflow diverter stent. Complete andnear complete occlusion was achieved in 82.6%. Intraoperativeperforation of aneurysms occurred in 19 procedures (4.3%).Thrombotic complications occurred in two procedures (0.4%). Braininfarction occurred in 52 patients (11.6%). Regarding functionaloutcome (mRS at discharge), 379 patients (84.7%) had goodoutcome (MRS = 1–3) and 68 patients (15.2%) had poor outcome(MRS = 4–6).
