Abstract

Before February 2008, all TNTS operations performed at our institution were done using a speculum and microscope (SPEC). Subsequently, all were performed using a dual-surgeon endoscopic approach (ENDO). The endoscopic approach has several potential advantages, including improved direct visualization plus less constrained bimanual dissection, thereby facilitating a more complete resection and obviating “blind dissection,” the latter resulting in intraoperative CSF leaks. A potentially negative consequence of more aggressive tumor removal could be a higher incidence of pituitary dysfunction postoperatively.
