Abstract

Hyperplasias of the endometrium progress to endometrial adenocarcinomas in about 12% of affected women. This study analyzed endometrial histopathology in 245 consecutive Sri Lankan women (mean age 55.8 years) who presented to government and private hospitals throughout the country in 1994-96 with postmenopausal bleeding. None of the women was on hormone replacement therapy. Endometrial samples were obtained through dilatation and curettage. 14.8% of specimens did not contain sufficient tissue for histologic assessment. Endometrial atrophy was present in 29.4% of specimens. 7 women (2.8%) had adenocarcinoma of the endometrium and 12 (4.9%) had cervical cancer. Hyperplasia with and without cytologic atypia was present in 8.9% of specimens. Endometritis was found in 5.8%. These findings confirm the varied etiology of postmenopausal bleeding. Availability of endocervical ultrasonography in Sri Lanka would make investigations of postmenopausal bleeding more efficient.
