Abstract

ABSTRACT A male infant with severe retardation and hypothyroidism, his goitrous and cretinous mother, and his maternal grandparents, both hypothyroid and with goiter, have been studied. The family resides in rural Andean Ecuador where endemic goiter is severe. Thyroxine concentration in the serum of the grandfather was below the normal range, and levels in the grandmother, the cretinous mother and the infant were very low. Serum thyrotropin concentrations were high in all four. The lack of clinical suggestion of cretinism in the infant indicates the urgency of early laboratory diagnosis in similar circumstances; the situation, as a whole, points to the compelling need for preventive measures.
