Abstract

To the Editor: Schnitzler syndrome (SS) is characterized by recurrent high fever, urticaria, and bone pain concomitant with IgM monoclonal gammopathy of undetermined significance. We report a case of SS with an elevation of serum interleukin-18 (IL-18) and monoclonal IgM-positive B cells that express messenger RNA for IL-18, but not IL-1, in bone marrow. The symptoms were all alleviated by rituximab, an antibody against CD20.
