Abstract

The aim of the investigation was to examine whether a long-term periodontal stability can be achieved by closed periodontal therapy with ultrasonic and regular supportive periodontal therapy (SPT) in a dental practice. Patients diagnosed with chronic periodontitis who had undergone systematic periodontal treatment with ultrasonic (Piezon Master 400 ®, EMS) within the meaning of a full-mouth scaling and at least three years in SPT. The initial findings were collected from the patient records (age, gender, smoking habits, medical history, recall rate), periodontal (dental status, probing depth) and panoramic radiographs (bone loss). In a clinical follow-up was the collection of the temporary tooth loss, probing depth, and using current panoramic radiographs of bone loss. Statistical analysis: nonparametric analysis of variance ANOVA (significance level: p <0.05). From 193 potential patients were 92 (w = 54, m = 38) with an average age of 55 ± 12.6 years are included. The average follow-up period was 6.7 ± 2.1 years. During the observation period a total of 77 teeth were extracted (54 from periodontal reasons). Only age showed a significant effect (p = 0.01). The average probing depth was reduced from 4.1 ± 1.2 mm to 3.0 ± 1.2 mm. Divided by groups of teeth, showed front teeth, smoking habits (p = 0.004), medical history (p = 0.051) and gender (p = 0.026) and maxillary molars age (p = 0.0004) and gender (p = 0.020) has a significant influence; for premolars and lower-molars only the smoking habits (p = 0.002). The bone loss remained constant over the observation period (3.4 ± 1.6 mm to 3.3 ± 1.5 mm). In all tooth groups had only age significantly affected (p = 0.001). The patient risk of periodontal disease are significantly reduced by treatment and years of follow-up. In the "low risk" group been counted at "base! " 12 (13.04%) patients as of "end " there are already 42 patients (45,65%) to have a low forecast of a new disease. In the "medium risk " group decrease the number of patients from initial 63 (68.48%) to 43 (46.74%). The number of "high risk" notes of 17 patients (17.48%) to 7 (7.61%). A similar effect is achieved, by therapy and SPT to the tooth forecasts. The result: in the group of front teeth 66% of the teeth to improve their prognosis, 64% in the group of the premolars, 51% in the group of the maxillary molars and also 51% of the mandibular molars. The results of this study show that can be achieved in the presence of chronic periodontitis by ultrasonic therapy and consistent follow-up (SPT) long term periodontal stability.
