Abstract

Objective To explore the effects of transitional care on adolescent patient with Type 1 diabetes (T1DM) after discharging from hospital. Methods The study investigated 78 adolescent patients with T1DM and they were randomly divided into observation group and control group on average. The patients of observation group received the transitional care and the patients of control group received the routine care and telephone follow-up after discharging from hospital. The self-management ability, glycemic control and incidence rate of readmission in six months after discharging were compared between the two groups. Results The patients of observation group showed higher self-management efficacy and behavior scores and lower HbA1c level than the control group (P 0.05). Conclusions Transitional care is an effective method to improve patients′ self-management capacity and glycemic control after they discharge from hospital.
