Abstract
目的 研究异丙酚麻醉与普鲁卡因静脉麻醉比较对颅内压的影响.方法拟于全麻下行脑瘤手术病人60例,ASA分级Ⅰ～Ⅱ.随机分异丙酚麻醉组(D组)和普鲁卡因麻醉组(P组).入室后连续监测HR、ECG、BP、SP02.L3～4间隙腰大池穿刺,置管,接压力传感器.全麻诱导插管,术中D组使用微泵持续注射异丙酚维持麻醉,P组使用1%普鲁卡因静脉滴注维持麻醉.外科医生开硬膜前及术中询问满意度,以0～100记分.测量平卧后、开硬膜前、开硬膜后的ICP.结果两组病人ICP平卧后D组与PI组比较无显著性差异(P＞0.05);开硬膜前D组与P组比较有显著的统计学意义(P＜0.001);开硬膜后D组与P组比较有显著的统计学意义(P＜0.05);神经外科医师满意度D组与P组比较有显著的统计学意义(P＜0.001).结论本实验证明在降低神经外科手术ICP方面异丙酚静脉麻醉优于普鲁卡因麻醉。
