Abstract

Objective The accumulation of serum parathyroid hormone (PTH) plays an important role in the pathogenesis of dialysis-related secondary hyperparathyroidism (SHPT).Both high-flux hemodialysis (HFHD) and hemodiafiltration (HDF) can effectively remove PTH.This investigation was to compare the efficacy of HFHD + HDF vs HFHD or low-flux hemodialysis (LFHD) on the removal of PTH.Methods A total of 36 patients who underwent maintenance hemodialysis were randomly assigned to the HFHD+ HDF group (n =12),the HFHD group (n =12) and the LFHD group (n =12).Nipro150DS,HF80S and Gambro's 14L dialyzer was used for HFHD,HDF and LFHD,respectively. After 12 months'treatment, serum calcium, phosphorus,PTH,alkaline phosphatase (AKP),BUN,Scr and albumin (ALB)were observed. Results All the three groups showed no significant changes in serum calcium before and after the intervention.Serum phosphorus of the HFHD + HDF group was slightly declined.PTH of both the HFHD + HDF and the HFHD group was decreased to some extent. Conclusion Both HFHD + HDF and HFHD could reduce serum PTH of patients who are receiving maintenance hemodialysis. Key words: High-flux hemodialysis; Hemodiafiltration; Maintenance hemodialysis; Parathyroid hormone
