Abstract

Rationale: Hospital stays can deteriorate older adults’ health and nutritional status (NS). At discharge, prevalence of malnutrition (MN) and risk of MN are still high among older patients, indicating a requirement for ongoing care. This study aims to evaluate whether an individualized comprehensive health advice by “pathfinders” (PF) after discharge may improve their NS.
