Abstract

Objective To investigate the effect of progressive decompression surgery regimen on neurological function, intracranial pressure and prognosis in patients with severe traumatic brain injury (STBI). Methods 88 patients with STBI treated at our hospital from January, 2015 to June, 2017 were divided into two groups by draw lots with 1:1. The observation group were treated with improved large bone flap progressive decompression, while the control group with improved large bone flap routine decompression. The intracranial pressure, volume of hematoma, neurological function, activity of daily living, prognosis, and complications were compared between the two groups. Results 1 and 5 days after the surgery, the hematoma volume and intracranial pressure in two groups were lower than those before the surgery, and were higher in control group than in the observation group (P 0.05). Conclusion Improved large bone flap progressive decompression surgery in the treatment of STBI can control intracranial pressure, reduce hematoma volume, increase neurological function and activity of daily living, decrease the incidence of complications, and improve prognosis.
