Abstract

Objective To observe the heterogeneous response of oxgen metabolism and gastric mucosal pH of acute respiratory distress syndrome (ARDS) piglets caused by pulmonary and extrapulmonary insults to restrictive or non-restrictive fluid therapy.Methods 24 piglets were randomly divided into ARDSp (caused by hydrochloric acid lung lavage) group (group A) and ARDSexp (caused by oleic acid injection) group (group B).In group A or B,piglets were randomly treated by restrictive ( group A1,B1 ) or non-restrictive (group B1,B2) fluid therapy for 6h guided by EGDT.The results of oxygenation index and pH1 were measured on different time points.Results After ARDSp/ARDSexp models were established,arterial oxygenation index reduced to below 200,and pH1 got worsen obviously ( < 7.32 ).Afer the 6h fluid therapy,arterial oxygenation index exhibited improvement in Group B1,All piglets exhibited improvement of pH1 but the effects were less significant in Group B1.Conclusion Restrictive fluid therapy can help to improve oxygenation and reduce pulmonary edema in ARDSexp,but may result in organ hypoperfusion.
