The background papers underwent several rounds of revision over about 5 months, based on comments by independent peer reviewers and an Annals statistical editor. The background papers and the recommendation statement were all in final, accepted form by 10 September 2009. Annals scheduled them for the next available print issue, which was the 17 November 2009 issue.
