Abstract

This study looks at the response to oxybutynin therapy as a means of diagnosing infravesical obstruction in a group of boys with urinary incontinence. First, I am struck by the number of oddities in the group studied by these authors. The number and percentage of boys with bladder outlet obstruction was very high (75%) and the positive response in the boys with an obstruction was also very high (76%). The incidence of infravesical obstruction in most large series varies between 5% and 15%. Can the authors explain why their study population had so many boys with obstruction? In addition, a 60% incidence of posterior urethral valves as a cause of obstruction as reported in this series is remarkable. I would want the authors to spell out their exact criteria for diagnosing this condition.
