Tenets of fistula repair are a good tension-free closure with healthy tissue and especially for “complex” fistulas, an interposition flap. A peritoneal or omental flap is relatively easily attained abdominally, but not so easy vaginally where a peritoneal flap can be found if you are fortunate or a Martius flap can be used if the fistula is distal.
