In my anecdotal experience of operating in a major trauma centre in northern India where we were performing unreamed tibial nailings in open grade III B fractures primarily and after initial external fixator applications, we found the results were encouraging. I fully agree with the authors that clinical trails of more such cases, prospective trials or multi centre studies are needed to ascertain the effectiveness of this regimen.
