Abstract

Purpose: to report a case of early postoperative capsular block syndrome after routine cataract surgery. Method: case report. Result: A 80-year-old woman underwent uneventful phaco surgery in her right eye with posterior chamber intraocular lens implantation after continuous curvilinear capsulorhexis and viscoelastic device (Viscoat) usage. On the first postoperative day, she had an unexpected myopic shift. This persisted for 5 weeks until a distended posterior capsular bag was noticed through a dilated pupil. Under the impression of capsular block syndrome (CBS), Nd: YAG laser anterior capsulotomy and posterior capsulotomy were performed. The above signs were resolved immediately. Two weeks later, the myopic shift was greatly reduced. Conclusion: CBS is a rare complication of continuous curvilinear capsulorhexis with retention of viscoelastic material. In our case, early postoperative CBS did not resolve spontaneously until Nd: YAG laser capsulotomy was carried out.
