Abstract
Great collaborators in history—like Orville and Wilbur Wright, Watson and Crick, and Simon and Garfunkel shared a passion for innovation and creativity. So too do Samaneh T. Wilkinson, PharmD, MS, Assistant Director of Pharmacy at the University of Kansas Hospital in Kansas City, KS, and her boss Rick Couldry, BSPharm, MS, Director of the Pharmacy Department. Together, this pair has forever changed pharmacy practice at the hospital by implementing a pharmacist-facilitated medication reconciliation program where 95% of patients have direct face-to face interactions with a member of the pharmacy staff to discuss their medications.
