Abstract
Endoscopic submucosal dissection (ESD) and endoscopic mucosal resection (EMR) of polyps at the Ileocecal valve (ICV) can be challenging. The ICV anatomical location can result in poor endoscope access, poor positioning of the endoscope tip and proximal scope difficulties such as looping, decreasing scope control. ICV polypectomy is associated with a poor outcome. Here we report the first ever case series of ICV polypectomy using a double balloon oversheath platform. We postulated that use of the device may overcome some of the challenges of ICV polypectomy. 2 cases were included in this case series, both elderly males referred for colectomy. In both cases, polypectomy with device assistance was successful. The procedure was less challenging qualitatively due to increased endoscope stability, 1-to-1 movements of the scope tip improving precision and improved endoscope movements in retroflexion. In conclusion, this approach may be considered by clinicians to overcome some of the challenges associated with ICV polypectomy. Watch the video: https://goo.gl/vuUJHA
