Abstract

Statement of the Problem: The purpose of this paper is to describe the sandwich osteotomy for correction of vertical maxillary alveolar bony defects and compare the results to that of traditional distraction osteogenesis. Correction of the vertical defect is typically performed for those patients with high smile lines and aesthetic concerns. Onlay bone grafting has been fraught with problems of immediate and delayed dehiscence with subsequent partial or total loss of the graft. Distraction osteogenesis surfaced as one solution. Experience has revealed vertical relapse and palatal displacement of the transport disk, often leading to subsequent labial onlay bone grafting.
