Abstract
The impetus prompted by the NHS and Community Care Act 1990 means that acute trusts now regard the discharge planning process as a collective activity with each professional discipline making its own contribution. Multidisciplinary involvement is the key to successful discharge planning. Early referrals to community nursing services, social services, physiotherapy or occupational therapy will promote consideration of patients' medical, personal, functional and social circumstances.
