Abstract

As the elderly population increases, the importance of treatment and management of elderly diabetic patients is also growing. Diabetes is a disease for which self-management is important; diabetes educators form a team to provide education and counseling to patients that will improve lifestyle habits and self-management skills. However, for elderly diabetic patients who are expected to have difficulty in self-management, a discharge plan that considers medical, psychosocial, and economic aspects as well as education should be developed. Through such planning, continuity of effective diabetes management and treatment can be ensured even after discharge.
