Abstract

Hospital readmission has long been a threat to our country’s healthcare system. Sepsis has recently become the number one disease condition resulting in readmissions following an initial inpatient hospital discharge. Increase in the number of sepsis survivors causes a huge burden to the healthcare system secondary to increased morbidity, mortality, utilization of health care resources, and transition to hospice. This integrative review explored the existing literature on the effect of post sepsis care education on reducing all cause readmission among sepsis survivors.
