Abstract

The development of paraparesis in the setting of back pain and lassitude in an immunocompetent patient was diagnosed as the presenting feature of diffuse large B-cell lymphoma. MRI imaging and surgical biopsy were diagnostic, despite the absence of lymphadenopathy or splenomegaly, and near-normal laboratory workup. Different possible pathological mechanisms of paraparesis in large B-cell lymphoma are discussed.
