Abstract

Ischemia reperfusion (IR) injury is an unavoidable consequence of lung transplantation. Reperfusion of lung grafts induces physical stress and biological responses to recipient blood. Ex vivo lung perfusion (EVLP) can be considered to be a “modified reperfusion” that simulates the physical aspects of IR, by perfusing the lung with an acellular solution. By comparing the responses of implanted lungs versus those put on normothermic EVLP, we can distinguish the physical and biological responses to IR. We can also identify responses exclusive to EVLP.
