Abstract

The cause and prevention of the long-term complications of diabetes remain a major puzzle. It is still unclear whether the microvascular lesions of diabetes are a genetically determined and integral component of the disorder, or the secondary outcome of abnormal metabolism. There is no conclusive evidence of histologic changes, in particular capillary basement membrane thickening, in so-called prediabetics, potential diabetics, or diabetics examined soon after diagnosis. Capillary basement membrane thickening has been reported inpotential diabetics, i.e., offspring of conjugal diabetics (9) but these findings have not been confirmed (12). Similarly, thickening of the glomerular capillary basement membrane has been reported in recently diagnosed and prediabetic patients (8,12) but Osterby Hansen in a careful study, (6) found no difference in glomerular capillary basement membrane thickness between recently diagnosed diabetics and nondiabetics.
