Abstract
Persons living with diabetes are more likely to have longer hospital admissions and are at increased risk for readmission, compared to those without diabetes. Admission to hospital presents a unique opportunity to provide innovative diabetes management support. To promote high-quality diabetes care while shortening length of stay (LOS) and decreasing readmission rates, the impact of a diabetes clinical practice leader (CPL) role was assessed. This specialized role involves 1) providing diabetes survival skills education and facilitating timely follow-up with outpatient diabetes services; 2) upskilling frontline staff with diabetes-related content; 3) consultation to optimize diabetes treatment plans; and 4) organization-wide quality improvement initiatives.
